Get the camera set up nice and level on the tripod, take a few practise shots to get the right exposure, obviously you'll need all the photos to be exactly the same otherwise it wont blend. Use a narrow aperture, something like f/8 so that your lens will be around the sharpest it can be (most lenses are sharpest between f/8 and f/11).
Once you're happy with everything, work out where you're going to start off, and how many photos you're going to need ie. 3, 4, 5 etc. Most pano's are normally made up of 3 photos.
You need to look for objects in each photo so that you can memorise where the photo previous to it ended, and you need to overlap it by about half an inch in the view finder.
Sounds complicated as fuck, but it's really not. I'll do you a diagram in photoshop to explain what I mean, it just takes a little bit of practise to get one perfect. The longer you spend setting it up, the better your result will be.
